EXERCISE NO. 3 JOINT AFFIDAVIT RICARDO B. MENDOZA, is a resident of Brgy. Ilat South, San Pascual, Batangas and the son of Sps. CRISOSTOMO MENDOZA and MARTA BULANHAGUI. He was born on JULY 4, 1941 at Brgy. ILAT, BAUAN, BATANGAS (now Brgy. Ilat South, San Pascual, Batangas). He is the fourth child of the spouses after: Nenita, Pepito and Paciano. His facts of birth were registered and recorded with the Local Civil Registry of Bauan, Batangas. He has indicated in all his personal records, including his Certificate of Marriage, Social Security System Records, Government Service Insurance System, Comelec Identification Card, Driver, License, Senior Citizen Identification Card and all other records, that his date of birth is MAY 4, 1941. He is applying for a Philippine Passport and is being required by the Department of Foreign Affairs to submit a Joint Affidavit of two disinterested persons to attest that RICARDO B. MENDOZA, born on JULY 4, 1941 and RICARDO B. MENDOLA, born on MAY 4, 1941, as per documents enumerated above, pertain to one and the same person. He was able to ask MACARIO L. ILAGAN and DEMETRIA C. BOO, Filipinos, both of legal age, and residents of Brgy. Ilat South, San Pascual, Batangas, to execute the Joint Affidavit. They exhibited the following identification cards: MACARIO L.
