Exploring the Speed Bumps and Pitfalls to Project Implementation. Explain Honestly, the clinical team and I engaged in multiple discussions to identify potential factors that might present an impediment to implementing the project, but our efforts yielded no evidence of threats. The greatest concern for us was at the level of the patients' interdisciplinary team, as it relates to their potential unwillingness to work as a collaborative team member. To date, we have not experienced any challenges at the site level that have stymied the implementation of the project. We have transitioned to the monitoring phase of the project and feel confident that the evidence-based measures and tools we've implemented to close the practice gap will be highly effective
