you will need to relate your answers to the practices in each section with an international agreement or statement.
For example, one that relates to discrimination is the Convention of the Rights of People with Disabilities and the Declaration of Human Rights. You may end up using a specific declaration for several practices but there are some that are very specific for that practice.

This is where you would put federal and state acts that relate to the practice.
For example, Age discrimination Act 2004, Disability Discrimination Act 1992 etc
Column four:
This is where you would discuss how you apply these acts to your work. Focus is on wellbeing of the client
For example, with discrimination you might say these laws seek for worker to apply anti-discrimination actions when supporting clients such as giving respect, acknowledging their cultural and ethnicity when including them in decision making processes. As a worker it is a responsibility to ensure that there is no discrimination for the client, their families and other workers and members of the public.
