For this step you and your partner will research possible reasons that this deficiency is occurring. In a real office situation you would interview the people who use the data to come to conclusions; since you don't work in this office, we can't do this part. But we can think of various causes. You will then select one cause (basically you're pretending that this is the main problem causing the deficiency) and create a performance improvement plan to correct the problem that is making it occur, and prevent it from happening in the future. To begin, you'll think about the workflows that might happen in different departments to create just one record, or to get a signature on a record, or to populate the record (whatever deficiency you're looking at). Think about the people, policies, technologies and management that affect the information that goes into a record. How might these things affect the quality of the record?
