Fortis Hospitals, Bannerghatta Road, Bangalore conducts executive health check-up for the employees of IIM Bangalore. The employees are requested to take appointments in advance and visit the hospital for an executive health checkup(EHC). Normally EHCs are scheduled for Saturdays only. While the executive health checkup is done by a separate wing, several tests such as TMT, ECG, and X-Ray are done using the common facility shared by other divisions of the hospital. There is afront office staff at the EHC unit who receives the customers, completes the registration formalities, attends to paperwork and directs the customers to various steps in the health checkup plan. She is assisted by three more people. The service is designed in such a fashion that the EHC center works for five hours from 8.00 am to1.00 pm. It is expected that all patients will finish EHC and go home for lunch. The EHC process is as follows. First, an arriving customer completes the registration formalities by interacting with the front office staff at the EHC wing. Then he/she is asked to give the first sample. Then they have breakfast as the second sample has to be given two hours after having the breakfast. In between the other tests are completed. While half the group follows a routing of ECG - TMT and X-Ray the other half follows X-Ray - TMT and ECG. After completing all the tests, the customer finally returns back to EHC and has the final consultation with the physician. The physician would have had most of the reports by then. Typically customers on average end up spending about 5 to 6 hours to complete the whole process. Many complain that a lot of waiting happens at a few stages in the process. Some customers give up the EHC routine once they reach the TMT as they feel more delays are likely. The actual work content (94 minutes) is a fraction of the total time they spend. It consists of the following: Sample 1 collection (7 minutes),TMT (30 minutes), ECG (10 minutes), X-Ray (10 minutes), breakfast (15 minutes),sample 2 collection (7 minutes), and final consultation with the physician (15 minutes). The two-hour constraint between sample 1 and sample 2 is another aspect to be considered. 3 ECG machines, 4 Treadmills (for TMT), and 5 X-Ray machines are available for the use of the hospital. There are two people in the sample collection unit, but one of them works on a half-time basis as she assists another section also. Three physicians are available for final consultation. Here is some more data you collected as a part of this assignment: The arrival rate of customers: 15 per hour (Poisson distribution). Arrivals happen only between 8 am and 10.00 am. Service rate at EHC: 15 minutes (Exponential distribution) Utilization due to demand from other divisions at ECG - 50%; X-Ray - 40%;TMT - 25% All the test facilities are assumed to be close by and walking time between them can be considered negligible. Assume that the COO of Fortis Hospitals has hired you for providing some advice. Provide a one-page report on what are the possible sources of problems and possible actions that they can take to bring down the total time in the system to less than 3hours.
