Go to YouTube and watch the "Expert Program Management" video on the Root Cause Analysis process known as "Five Whys"

Then go to the ASQ site and review two articles:

"Five Whys and Five Hows" "Fishbone diagram" Download the template for a Fishbone diagram at the end of the article. Take detailed notes during the video and conduct additional research for a thorough understanding.

Apply the Five Why + Fishbone Diagram frameworks to analyze a familiar work or personal issue. Use the Five Why technique, without limiting to a specific number, to drill down and identify the root cause. Explain the selected scenario, providing a brief background and rationale. Describe symptoms displayed by the initial issue. Utilize the ASQ Fishbone Diagram template to map out analysis categories. Clearly identify and discuss the root causes derived from the analysis. Copy and paste the completed fishbone diagram from the spreadhseet into the essay/report. (You may need to paste it as a picture to prevent formatting issues). Provide a concise explanation of the analysis process in a few paragraphs. Include a conclusion that highlights insights gained from the activity. Note: If you are using a work example, please do make sure you are allowed to share the details in our class forum (i.e. make sure you do not share any confidential and/or proprietary information).

The essay/written assignment must follow APA format. Include a title and reference page in the essay (include your name on the cover page). Do not include an abstract and table of contents. Incorporate and use section or paragraph headers for clarity. The essay must be at least 3 pages, excluding the title and reference pages. Clearly cite references within the text following APA style. List all references on the reference page. A minimum of one reference is mandatory for the assignment.
