Here are two Safety Reports. You are the SMS Manager. Take them through the Process Flow in a format you can present to your CEO, Director of Flight Operations, and Maintenance Manager in a Safety Meeting.
Safety Report 1 Hard Landing at CYYC
Landing on runway 15 at the Calgary International Airport, on Nov 3,2023 at 2:20 in the afternoon, I encountered a hard landing. I do not believe any damage was done to the aircraft and no injuries to passengers, however, a number of passengers commented that it was a very crunchy landing.
Boeing 737800 NG
Captain Michael Kennedy flying pilot
First Officer Neil Dobbs
Chinook Winds were from the west 280 deg at 25 knots gusting 35 knots
Upper winds were 270 deg 55 knots
Moderate to Severe turbulence during all phases of the descent.
Due to tailwinds we overflew the top of descent.
Possible maintenance inspection required before further flight.
Safety Report 2 FOD (Foreign Object Debris) CYOW
During a Runway Inspection this afternoon at the Ottawa airport we discovered a piece of aluminum metal on the threshold of Runway 27, about 3 cm long and 1 cm high. It had a serial number stamped on it VG-657933. It appears to be a vortex generator that may have fallen off another aircraft.
As we haven't done "Safety Investigations" yet, feel free to an educated/creative guess for that portion. I will be very easy on that part of the marking.
