History of Present Illness:

A 28-year-old female presented with low back pain, which began following her last pregnancy approximately 8 months previous to presentation for care.

ROS The review was unremarkable except for genitourinary symptoms including increased frequency and increased difficulty holding her urine. She reported nocturia times 2. Surgical history was significant for oophorectomy in 2006. Social history included drinking very little water, and consuming 3-4 large cups of coffee and 2 sodas daily. Family history was remarkable for diabetes, hypertension, heart disease, ulcers and osteoarthritis.

Pertinent Findings: A focused investigation of the low back was performed with the following results: decreased lumbar extension and left flexion with referral of pain to the right SI joint; Kemps positive on the right for pain at L5 right; Milgram’s positive for pain in the right SI joint; Ely’s positive bilaterally, with referral of pain to the left SI joint; tenderness inferior to the xiphoid process and at McBurney’s point.
