estimate and evaluate the probability of error for a given scenario. The deliverable for this
assignment is a written report. The maximum length for the report is three pages. The following
steps will lead you through the assignment. Be sure to address each step//question in your report.
1.1. Choose one of the following scenarios:
a.. Janet is a nurse at County Hospital. Her husband was recently laid off so she has
been working 6060 hour weeks to earn some extra income. On a recent night shift,
Janet delivered the typical medications to her patients rooms. The medications
are prefilled, labeled, and sorted by the hospital pharmacy. Patient Room 21122112
contained two patients, Mr.. Brown and Mr.. Smith. Janet inadvertently dispensed
the wrong medication to each patient, giving Mr.. Brown Mr.. Smiths medication,
and vice versa.
b.. Mike is a delivery driver for FedEx. He drives around town, delivering packages
to local business and homes. The current package Mike is delivering is supposed
to go to Ms.. M.. Mouse at 114114 Daisy Drive. However, Mike delivered the
package to 112112 Daisy Drive. The package did not require signature confirmation,
so Mike simply left the package on the front porch.
c.. Mary works in a manufacturing plant in Smallville. Her job entails the assembly
of 33 small pieces to create a widget, which is then sent further down the assembly
line. Mary sometimes forgets to include the 22nd piece of the widget. There is
currently no specific order in which it must be assembled, and it is not visibly
noticeable when the 22nd piece is missing.
2.2. Formulate this scenario as a THERP model. At each branch in the model, estimate the
probabilities of occurrence.
3.3. Based on your model in part 1,1, what is the overall probability of error?
4.4. Suggest two design improvements that would reduce error in this scenario.
5.5. Develop new estimates of your THERP probabilities based on your design
improvements. What is the new overall probability of error?
6.6. What level of error would you define as acceptable for this scenario? Why? Would your
acceptable level of error be different for the scenarios you did not choose?
