I am presented with a 7-year-old client who has been referred to counseling due to abnormal behaviors displayed in school. The client's aggression is unpredictable in school. Generally, he gets along with his classmates, he is not disruptive, and is often the first to finish his work. During playtime in school, he likes to play alone. During playtime he ignores and tunes out other children. Client lashes out and hits children if they continue to pursue him during playtime when he wants to be alone. He is at risk of being expelled.

According to DSM-5, without a further review of the case, V55.4 - Educational Maladjustment and Discord with Teachers and Classmates is the most appropriate diagnosis at this time. Considering the observed behaviors, especially the tendency for unsociable play and challenges with social connection during playtime, it may be wise to investigate the potential of coexisting illnesses like minor intellectual disability or autism spectrum disorder (ASD).
