I am writing a critical incedent report.

Please make it look proffessional, and here are details of what happened.

1000 Monique was still asleep when this shift commenced.

1015 Monique woke up, went to the toilet.

1019 Monique had her medication, administered by Joseph as prescribed.

1025 Monique started making calls to friends while showering.

1045 Joseph called Monique's doctor to let them know she was to be late as she was still having her personal care/shower.

1055 Monique cancelled her doctor's appointment, she then told Joseph to take her to the train station later. Joseph asked if it was still okay with her if she waits for at least two hours because there was astaff meeting going on, she said yes "I am not in hurry just take your time".

1120 Monique wanted to be taken to Cleveland train station, and Joseph asked her to wait for 10 minutes before leaving the staff meeting, Monique insisted as she wanted to be dropped off immediatly. Joseph therefore told her that it was okay for her to wait for 5 minutes then, she said no, let us go if not I will smash this house or destroy stuffs here. Monique smashed the office door and the next one, while going to the kitchen to grabb the knife, she said the knife will kill herself and she thretned to kill Joseph as well and whoever who will come around. When she was saying all those things, she called the police.

Joseph tried to stay or wait at the front lounge, but Monique said you have to go to the office or else I will sturb you with the knife, which she had in her hand. Joseph went in the office as instructed by Monique. After finishing the call with the police, she made some other calls which Joseph could not tell.

1135 The police arrived, searched the office trying to see if someone was hurt by Monique before attending to her.

Joseph mentioned that Monique missed her meds for two days which could also escalate the situation.

Police officer asked some questions afterwards and then attended to Monique.

1200 Monique left placement to the hospital, escorted by the Police for further check up.

1206 Joseph called Kelsie the case manage to inform her of what happened with Monique.
