I need an expert opinion for this case.

Dad brought his daughter, Jade, to the clinic today. Jade was last seen last month for a well-child check. Apparently while Jade was out in the backyard helping her father plant spring flowers in the garden, she stuck the stem of a dandelion flower up her nose, and the bud become lodged in her nasal cavity. Dr. Hayler was on call today and attended to the encounter. Dr. Hayler spent 35 minutes soothing her before he was able to look up her nose. Using blunt-tipped forceps, her removed the stuck flow bud. Jade and her father were relieved and left with the flower bud in a specimen jar. Total time spent was 45 minutes. Code the appropriate codes for the encounter!

Hint: No CPT Code for this visit. There's been debate in the class about why we would not code the surgical removal code for this, as they used a forceps. In this case, they just plucked something out of the patient's nose. Very little time and not really a "separate" procedure from the E&M. So it can be included in the E&M visit. The doc really just assessed the patient and plucked it out (under 10 minutes) but what took the time was trying to calm the patient down. How do we code this visit?

This is an outpatient office visit that took much more time than it should have for simply plucking a stem out of a nose. We don't use the MDM Table to code this. How else can it be coded?
