I need help with a case study using information: A white 30-year-old female with mental health, and addiction problems, living in the Appalachian, lives in public housing, is unemployed, single mom, and no healthy social support, no religious affiliation. Use any additional information to fill in all areas. Use format: 1. Population and Social Problem Give a brief description of the target population and issues addressed by their field agency. Briefly describe the agency's approach to delivering services to clients and/or addressing the needs of the target population. Note what philosophy, model(s), and/or rationale supports the agency's approach to client services.
2. Client Introduction Briefly describe a client/client system that you have begun working with or have observed. Reference that client/client system for the following sections of the paper. A basic description of the client system including client's age, gender, race/ethnicity, and nationality. Remember to change the names of clients and change or omit other identifiers.
3. Presenting Problem a. Nature of the problem b. Clients and significant others' (including SW student) view of the problem and are they compatible?
4. History of Presenting Problem Include any stressors that contribute to presenting problem, detailed history of treatment, what they have tried in the past, how symptoms have progressed, etc. 5. BPSS Assessment - This should include the following sections if relevant: Medical Include current or past medication conditions, medications, dosage, purpose, prescribing physician and allergies. You can include their current health status (good, fair, poor) and information about diet, nicotine and caffeine use, and exercise
Mental Health Include information about mental health conditions, diagnoses, or symptoms.
Family Include information on family of origin, and relationship with parents and siblings. You can include family history of mental illness or substance abuse. Note any family members who are deceased, how they died and the year of death. Note any history of neglect or abuse.
Developmental Include any developmental delays, milestones and you can include the stage of development from Erikson's stages of development.
Trauma History Include current and past traumatic events the client has faced. These could include poverty, abuse, neglect, loss of a loved one, or other major events that could be a source of trauma.
Spiritual Include information about religion, spirituality, participation in church or other faith-based communities.
Social Include information on social support, friends and the nature of those relationships. This can include school and early childhood relationships
Recreational Include any interests, hobbies, relaxation or fun activities.
Sexual/Relationship Include current and past significant relationships, sexual orientation, age of first sexual experience, any sexual abuse, any domestic violence and client's relationship with any children.
Legal Include any legal issues, probation and probation officer, current and past arrests and the outcome, any convictions and the outcome. You can include any bankruptcy or lawsuits.
Military Include whether they have ever been in the military. If they have, what rank, how long they were in it, if they are still in it and what type of discharge. IF CLIENT IS CHILD- EXCLUDE THIS SECTION
Education Include their educational history including if they graduated high school and what year. If they didn't, why they didn't graduate. Include any college or trade schools, major and if they finished. ADDRESS EDUCATION FROM THE DEVELOPMENTAL PERSPECTIVE- IF CLIENT IS A CHILD, TALK ABOUT PRESCHOOL, GRADESCHOOL, ETC. IF CHILD IS TOO YOUNG FOR PRESCHOOL, LEAVE SECTION OUT
Employment Include their past and current employment and satisfaction with employment. You can include whether they have any financial difficulties if they are underemployed or unemployed. DO NOT INCLUDE IF CLIENT IS A CHILD AND DOES NOT WORK
Substance abuse Include any information on substances abused past or present, age of fist use, date of last use, method of use, relapses and if they are in recovery. You can include information on caffeine and nicotine use here or with medical section.
Mental Status Note whether the client was on time, behavior, attitude, orientation to person, place and time, whether they appear to be stated age, whether they dressed appropriately for the weather and situation, mood, affect, tone of voice, rate of speech, judgment, memory, suicidal or homicidal ideation and any observable symptoms. 6. Assessment Process Describe the process of assessment with your client. Identify the sources of information used to complete the assessment. If important information was missing, identify this and what, if anything, you would do to obtain missing information. a. Present an assessment of the client and the presenting problem including relevant factors in the personal, interpersonal, and environmental dimensions. Include strengths as well as deficits, obstacles, and challenges in these dimensions. Information from a scholarly (peer reviewed) source about one or more client populations that are similar to your client (e.g., rural, diagnosis, divorced, etc.) will be useful here as an outside reference. b. Indicate the theory or other knowledge, learned in your courses, that helps you to understand the process and content in this session (e.g., what knowledge of human behavior or the human condition applies to this practice situation. Is information from Erikson, Maslow, Piaget, content from this course relevant)?
7. Goals and Objectives Discuss goals and objectives for work with client. Depending on your situation, answer either a or b below. Everyone answers c. Information from a scholarly (peer reviewed) source about the effectiveness of one or more types of intervention that have been utilized with the client population/issue similar to your client will strengthen this section. a. Students who have discussed goals and objectives with the client and who have established a formal or informal contract are asked to describe their steps in this process and make any suggestions about ways they might be more effectively in the future. Identify two goals, with two objectives each (specific, measurable, time bound). b. Students who have not set goals and objectives yet will discuss how they plan to carry out the process of setting goals and what factors they need to discuss with the client in order to establish a contract for their work together. Identify two possible goals, with two objectives each (specific, measurable, time bound). c. What intervention strategies could you use with this client? Why would they be a good choice?
