I need help with writing a tax notice letter.

Attached is a tax notice that I received from Ohio saying I owe them money. How could this be? I filed the return like you told me, though I'll admit I dropped the file you gave me carrying groceries into the house. It was a bit muddy by the time I sent it in, but I included the check for taxes due. I can't afford any more right now. What do I do?
