I would like you to research a few best practices for developing and/or administeringsurveys. There are lots and lots of tricks to not just developing a good survey but to increasing the chances that consumers will participate in the survey. As we study survey research isn't a DIY project any more than the other types of VOC methodologies we've learned about. My advice is to consult with someone who has experience in developing and analyzing survey research. There should be an analytical plan for each question that you ask in a survey. You should know why you asked the question and how you will use the results. For example will you compare women versus men, will you compare certain age groups? And why and how will you compare these two groups?Your answers dictate the format of the data you will collect (yes/no data, ratio data, or scale data for example).
