She is on a probation order for a drug possession conviction and has just been referred to an alcohol and drug service. She also was sexually abused as a child and subsequently self-referred to a counsellor recently. The following roles are needed:

1. The client
2. The case manager of probation and parole - it is agreed that this service holds primary case
3. Alcohol and other drug worker
4. Sexual abuse counsellor

Above four stakeholders meet to develop a case management plan. Develop the plan collectively, and submit a copy of this plan individually. Additionally, respond to the following 4 case management principles within the context of how the plan was assessed, developed, plans for implementing and how to review outcomes:

Assessment - What factors were consider necessary to determine when stakeholders met?

Planning - At the meeting to develop the plan, how were priorities consider and how was it ensured that the client was not overwhelmed?

Implementation - How was a step-by-step guide made to ensure the plan was collaborative and how did the 3 services come together to ensure the client's needs would be met?

Review - Was a review date established and how were outcomes to be measured? Were there clear goals with action steps included?
