I am looking for help in creating a more in-depth list of questions from the perspective of the LSO for direct questions and re-examining the lawyer and the medical practitioner.

Can you briefly explain the requirements of that section? Did Mr. Clark comply with By-law 9? How did he fail to do so? Is this a common error amongst licensees? How long have you been working as a Medical Practigner? 10 years What area of Medicine do you practice? Psychiatrist at the Centre for Addiction and Mental Health Toronto. I see patients in the centre, and teach in U of T. Mr. Clark's doctor has taken the stand and provided a medical note labeled exhibit (X). In your opinion, would this diagnosis hinder Mr. Clark's ability to maintain his records? From the medical notes that I received, I saw Mr. Clark had gastrointestinal problem and was diagnosed as esophageal reflux; his doctor has prescribed him medicine to control it. Mr. Clark also complained his anxiety but his family doctor did not refer him to a specialist. Usually family doctors refer patient to a specialist like me when a patient cannot get to work or maintain usual activities of daily living. So from the medical nots, I didn't see anything hinder Mr. Clark's ability to maintain his records. Would this diagnosis have any effect on Mr. Clarks ability to maintain his records?
