Consider the scenario listed in this module's W05 Foundation: Logic Model section, and construct a Logic Model for your local government as they battle this problem. Use the W05 Materials: CDC Program Evaluation Framework Checklist to guide the assignment.

You can brainstorm the Logic Model in many different ways. Some professionals use a whiteboard, or sticky notes on a wall, or computer programs to doo this.

Regardless of the visual properties, all Logic Models include the following elements: purpose statement, inputs/resources, activities, outputs, short-term outcomes, long-term impacts, and moderators.Some Logic Models also include arrows to indicate connections among elements visually.

  1. Develop a purpose statement to guide the Logic Model. The purpose statement should be aligned with the topic listed in the W05 Foundation: Logic Model
    1. List out all the components of the Logic Model (purpose statement, inputs/resources, activities, outcomes, short-term outcomes, long-term impacts, and moderators/barriers).

Assess the inputs (resources) available to your local government to address the W05 Foundation: Logic Model scenario and record them in the Logic Model under the heading "Inputs." The resources should be based on assumptions about the actual agency resources.

Consider the activities your organization should implement to address the prompt and record them in the Logic Model under "Activities." Activities refer specifically to the actions and programs your organization plans to implement.

Continue this process with the outputs, short-term outcomes, long-term outcomes/impacts, and moderators/barriers as listed in the CDC checklist.

  1. Writte a one-page paper explaining the Logic Model. The paper should be written in complete sentences and include additional details beyond what is included in the Logic Model.
  2. Submit TWO documents, the Logic Model, and a one-page explanatory paper.
