We have discussed many of the various sectors in the Canadian health care system. It is evident that each sector has various issues, and it is up to future leaders to collaborate to identify these issues and work towards solutions. We have also studied the various attributes of the Population Health Template and Model (Who; What; How). Using a minimum of three scholarly articles, choose one sector of interest from list #1 and using the Population Health Template and Model, devise a response plan using the questions from list #2. You may use subtitles to organizes your thoughts.

1. Choose one of the following sector options: Long-term Care/ Aging Population Sector Acute Hospital Sector Mental Health and Addictions Sector Home and Community Care Palliative Care Sector Indigenous Health Care Population 2. Answer all of the following questions: What is key information to introduce the sector? What are the identified issues within the sector?
