Religion: none IDENTIFIED PROBLEMS What are they? The client's parents report that he has shown a repeated pattern of defiance and disobedience both at school and home. The client does not believe his actions are out of the norm. He stated that everyone should stop complaining about him and that things could be worse. During the intake session the client repeatedly clicked a pen, even after his parents asked him to stop. When his mother removed the pen, he grabbed another off the desk, looked at his mother and clicked it. He was referred for counseling by the high school's psychologist. With the goal being to help him avoid being suspended or expelled while in high school. In what ways has the situation you are in affected your everyday life? (sleep, appetite, interests, intimacy, social life, finances). School The client was suspended 5 times during his 3 years in middle school. The primary reason for the suspensions were argumentative outbursts and deliberate breaking of school policy. The client's parents reported that the final suspension came during the last weeks of 8th grade when he said to the principle "go f**k yourself, I am outta here" Currently at the high school, he was most recently given an in school suspension for arguing with his biology teacher. He was shouting so loud that the school's security officer had to be called in. The client's grades have been slipping since 6th grade. He does not attribute his low grades to his own inaction but rather it is everyone else's fault. He repeatedly blamed his teachers as well as his parents for his failing grades. The client feels that the rules do not apply to him and actively defies them whenever he can. This has led him spending nearly every day in detention. Home The same pattern of behavior that is seen in school is also seen at home. The client is very argumentative and will slam doors, throw things, and shout when he is angry. When he is punished for misbehavior, his parents report that he will try to get back at them in some way. As an example, they stated that recently the client was not permitted to go on a ski trip due to his grades. The client's parents stated that while they were gone, he placed dog feces in all of their shoes, keyed both cars, and emptied every bottle of dish, laundry, and body soap in the household into the center of their bed. When confronted, the client told his parents that they should have let him go on the trip and it was their fault he got an "F" in biology and algebra. The client's parents reported that he had lost a few friends over the years due to his symptoms but still had several friends at school and online. How long have they been going on? (Onset date and progression) The client's parents reported that his symptoms began when he was around 10 years old. They stated that the incidents have worsened over the years but they are thankful he has never hurt anyone or been arrested. How often are they encountered? The client's parents reported that incidents occur several times a week. Do other family members share the view of the problem(s)? The client's parents reported that he saw the school psychologist during middle school but it did not seem to help. FAMILY HISTORY Briefly describe the family in which you grew up. The client's childhood was reported as being good. His mother stated that he was a happy baby and preschooler. She began to notice a change in his temperament when he was in elementary school. No marital discord or other pertinent life events were reported. Briefly describe your current relationship with your: Mother and Father - The client's relationship with his parents is very strained and he seems disconnected from them. The client reported that he doesn't hate his parents. He just wishes they would get off his back and let him live his life. Yes No - If yes, give details. The client's maternal grandfather struggled with schizophrenia and his father with depression. What organizations would you include in your support system? While his parents are supportive, the client reports that his support system only includes his friends. The client reports that he likes to play basketball with his friends and go to the gym to run around the track What would you want to accomplish during our work together? The client's parents reported that they would like him to be more respectful and be more calm and relaxed. The client stated that if these sessions could help him join the high school's basketball and/or track team then he would consider coming.
