Ironman suffers from mild epilepsy, and his symptoms are similar to "auras" also known as "simple focal seizures". These are not the type of seizures that cause loss of consciousness, but they affect motor symptoms and cause loss of control of feet, arms, and other parts of the body. Ironman has suffered from this condition since childhood but controls the effects of these auras through medication. With daily medication, the shaking or jerking movements of his feet are barely noticeable, and he remains fully functional and in control of his extremities and engages in all regular life activities. Ironman's doctor's prescriptions do not specify a specific time to take the medication, but Ironman has become quite adept at reading his body: he has figured out the best time to take his medication on most days is between 12pm-2pm. This way, the medication's effectiveness is strongest throughout the rest of the day. Additionally, Ironman can feel an aura coming on about a minute or two before it affects his hands and legs. Luckily, the effects of this medication are immediate, reducing his shaking within a couple of seconds.

One evening, after spending most of the day with friends, Ironman offered his co-worker, Black Widow, a ride home. That day, he took his medication in the early morning, because he wanted to enjoy the day and afternoon with his friends unabated. But that evening, as he was driving with Black Widow in the passenger seat, he started to feel uneasy. Convinced that one of these auras was coming on, he asked Black Widow to open the glove compartment and take the bottle of his medication out to give him a pill. Black Widow does not do so at first because she's too busy on his phone scrolling through TikTok with her earbuds in. Finally, Ironman screams at Black Widow to get her attention, but before she could locate the medicine, the aura came on, causing jerking of Ironman's legs that led to him to hit the gas suddenly. Ironman lost control of the vehicle and crashed into a tree on the side of the road. Fortunately, neither Ironman nor Black Widow was killed. Black Widow, however, was seriously injured with two broken legs.

Captain America, who was riding his bike on the same road, but across the road on the other side, tried to stop after hearing and seeing the impact of Ironman's car onto the tree. Captain America ran his bike onto the sidewalk, and ran into a child, causing injury to himself and to the child.
