FHC maintains information about patient appointments in the following table TREATMENTS (TREATMENT-NO, T-DATE, PROCEDURE-NO, P-DESCRIPTION, CHARGE, PATIENT NO, PATIENT-NAME, BILLING-ADDRESS, DOCTOR-NO, DOCTOR-NAME, SPECIALTY, NURSE-NO NURSE-NAME, FOLLOWUP-NO, F-DATE, TEST-RESULTS) The clinic wants to keep track of data for two different types of doctor's appointments: treatment appointments and follow-up appointments. These are separate entities in the database. The database should capture which doctor, nurses and patient are involved in each appointment A patient comes in for a treatment appointment and receives one or more procedures performed by one doctor, assisted by one or two nurses. Only one doctor treats the patient during an appointment and that doctor performs all the necessary procedures. The charge for each procedure depends upon several factors (time, drug amounts, etc.), so a procedure charge may be different each time it is done.A treatment can involve one or more types of procedures, and the same kind of procedure can b performed during any treatment appointment. (Billing to patients for total charges of appointments will be done later, so there is no cash and cash receipts included in this database.) Sometimes a treatment appointment requires a follow-up appointment, sometimes no follow-up needed, but there is a maximum of one follow-up for a treatment appointment. In the follow up appointment the patient sees a doctor (may be the same doctor they saw in the treatment appointment or may be a different doctor) and the doctor examines the patient and may take blood samples for a blood test. Sometimes the blood test is necessary; sometimes it is not. The lab is very efficient so testing is done immediately and the results are returned immediately. The only data about the blood test that FHC wants to store in this database is the results of the test. No nurse is present for the follow-up appointment. During the follow-up the doctor checks the patient and reviews the blood test with the patient, if a test was done, 95% of the time no further treatments or testing is needed. 5% of the time the patient must return for another treatment appointment Here is an explanation of the data elements.
