Jane went to see her PCP, complaining of dizziness and fainting spells about 1 month ago. The PCP made a referral to the PMHNP for a psychiatric evaluation & psychotherapy when Jane commented on a growing sense of avoiding the more "social" events at work and with friends.

Jane reports: Prior to about four weeks ago, I felt fine. About 4 weeks ago, I attended the annual awards banquet of my community recreation center.The center's director was scheduled to serve as the "mistress of ceremonies" at the banquet. "I was expected to give a few opening remarks, introduce the speakers, announce the award winners, and make the closing remarks.However, the director called me minutes before the ceremony began to say she was ill and that I would need to step in for her and just wing it as the "mistress of ceremonies" for the evening. When I heard this, I thought, "I am going to be sick." I began to pace and thought, "I'm wearing the wrong clothes. Oh, my God, what am I going to do?" I felt I would let the community down if I didn't "pull this off," so I went to the bathroom, washed my face with cool water, retouched my makeup, and went back out to the banquet room. As I walked to the podium, I felt the room closing; I felt like I couldn't breathe and couldn't hear anything around me, but I kept thinking, "Stay cool." I felt that I was using every ounce of strength I had to walk up the steps of the stage and take the microphone in front of all of those people." The next thing I remember, " I was laying on a cot in some back room and having a group of people hovering over me, asking me if I was OK." I had fainted on stage.
