John is a 57-year-old man who has been referred to you for intervention. He is currently being treated for depression and anxiety. He also is a habitual alcohol user and has consumed 2-3 litres of wine per day for the last 10 years often supplementing that with 375 to 500 mls of spirit (vodka, whisky usually whichever is cheapest). He is currently in uncertain accommodation having been evicted for non-payment of rent, he is homeless, in emergency shelters or in vacant buildings. Due to current health issues relating to alcohol effects upon his liver, his doctor is recommending detoxification and abstinence. John is not convinced he needs to abstain but recognizes the need for treatment at this stage. He is in receipt of a government pension because of an injury which has caused him to be unable to work in his old job as a labourer. He may be able to undertake training in the future if his current mental health and addiction issues improve.
