The ethylene oxide system was critical to production, and it was in bad need of repairs and maintenance. There had been several small leaks, which had been repaired on the fly. The bottom flange was showing signs of failure. And, the system did not fully comply with all safety requirements and, in the back of his mind, Jos$$ was concerned that someone might get hurt.

The plant had foregone the last two scheduled semiannual maintenance shutdowns in an attempt to keep up with production demands. Unplanned shutdowns along with rework due to increased quality problems had resulted in a higher$-$than$-$normal backlog. Despite the backlog, revenues were down due to lower market prices for the plant$$s products. Additionally, plant management had promised that there would be no additional slippage in production this year.
