They are slightly behind schedule and Rik decides he will break protocol a little and tension the lines a little quicker than the manufacturer recommends. As a result of this, multiple towers collapse. Luckily, no one is harmed but one tower did fall on a $250,000 generator owned by Magic Power Co. An engineer is hired and determines that there were three points of failure in the incident:

1. Another subcontractor called Moore's Moorings failed to properly tighten the support lines to the their anchors ("Failure 1")

2. A manufacturing defect in the steel beams of the towers (made by Emmet Levine & Co.) meant that they were not as strong as the project required ("Failure 2")

3. The additional load on the steel beams from tensioning too quickly exceeded the beams' strength and caused them to fail ("Failure 3")

The engineer also determined that preventing Failure 1 would not have prevented the towers from falling. The engineer could not determine whether Failure 2 would have occurred without Failure 3 or vice-versa
