Learning Outcome 5 You are to produce a written report, in sufficient detail, in relation to the lines of reporting in a health and social care setting. Reporting Procedure 1-000 National Center For one health and social care setting research the reporting procedures for safeguarding, including maintaining confidentiality. Your findings must include a description of the lines of reporting and responsibility in relation to safeguarding, protection and welfare. Firstly, Produce a flowchart to show the lines of reporting in a health and social care environment. This could be a nursery, dentist, doctors surgery or care home. Then summarise what it shows and explain how your chart may change in a different setting or why it would be the same. Then Get an example of a real-life health and social care recording and reporting policy (print it out) and annotate (this can be by hand) to explain what is says how the lines of reporting and this must be in relation to safeguarding. Choose 5 or more key areas that link to both safeguarding, welfare and reporting. Then for each of the following you need to explain: 1. What it involves in terms of reporting information 2. How it fits into the lines of reporting/ responsibility when safeguarding individuals 3. How it helps to safeguard the welfare of individuals 4. The boundaries (or potential boundaries) in relation to confidentiality 5.
