Case Study Facts . Older wheelchair - bound gentleman lived independently in rural Indiana next to his daughter who helped in his care. . She used a local nursing home for respite care while she took a vacation to Florida. . He was cared for in the nursing home all week and on Friday was transported to the VA for dialysis which occurred routinely every week Monday, Wednesday and Friday. . During this time period, the nursing home DON also went on vacation and left the ADON in charge. . At 4:30 pm on Friday, the bedside nurse called the ADON at home when she realized that the man was not back from the VA. . Nurse tried to call the VA but the dialysis clinic was closed. After texting someone at the VA, the assumption was made that the VA kept the patient over the weekend. . VA called the nursing home on Monday asking why he wasn't at dialysis. Police called. . The man was found unconscious, face down on the ground and off the porch on which the taxi driver had placed him Friday evening after dialysis. He died one week later
