Your form should have at least 5 items on it. Does the form evaluate behaviors, results, or traits, primarily? Provide an example and give your rationale for what category you believe the measurement item falls into. For instance, on my form, under item B-1, inclusiveness is about behavior - showing respect, promoting fairness, etc. are all behaviors that are being measured - no specific results are mentioned. On page 3 of my form, however, a space for writing out goal progress has been provided. This is designed to measure results, i.e. what outcomes have been achieved by the employee. Evaluate the form and answer the following questions: How complete is the form? What, if any components do you need to add? Explain the benefit adding the missing component might provide to the employees or managers in the organization the form is used by. Provide any additional comments you have about the usefulness of the form. For example, I believe the form I selected has some good descriptions of primarily procedural type behaviors but the rating scale is fairly general and subject to interpretation.
