What were the allegations by the employee?

What made the employer suspicious or otherwise motivated the employer to challenge the claim as fraudulent or false?

How was the matter resolved, and at what level - i.e., administratively by the carrier, at the court or adjudicatory level, or by agreement between employer and employee?

Was the matter resolved fairly and justly, given your understanding of the facts?
