Maintaining patient safety is a priority in all healthcare organizations. However, the continued rise in medical errors brings a lot of challenges and concerns to patient care, health outcomes, and safety. Medical errors can result from various factors, including human error, communication breakdowns, system failures, and lack of standardized processes. These errors can lead to adverse events, prolonged hospital stays, increased healthcare costs, and even preventable deaths $($Justinia et al$.,2021).$

One of the primary challenges in reducing medical errors is the complexity of the healthcare system itself. Healthcare organizations often have intricate workflows, multiple stakeholders, and a vast amount of data to manage. Effective communication and coordination among healthcare professionals, departments, and facilities are crucial to ensuring patient safety, but breakdowns in these areas can contribute to errors. Another challenge is the resistance to change and the adoption of new technologies. Healthcare organizations may face cultural barriers, lack of resources, or inadequate training when implementing new systems or processes to improve patient safety $($Justinia et al$.,2021).$

However, healthcare information technology $($HIT$)$ has emerged as a powerful tool in addressing these challenges and reducing medical errors. Electronic health records $($EHRs$),$ clinical decision support systems $($CDSS$),$ and medication management systems are a few examples of HIT solutions that can help standardize processes, improve communication, and reduce human errors.

From a policy and regulatory standpoint, several initiatives have been implemented to support the adoption of HIT and promote patient safety. The Health Information Technology for Economic and Clinical Health $($HITECH$)$ Act, enacted as part of the American Recovery and Reinvestment Act $2009$ incentivized healthcare organizations to adopt and meaningfully use EHRs $($HHS$.$gov, $2023).$ The Office of the National Coordinator for Health Information Technology $($ONC$)$ has also played a crucial role in establishing standards and certification criteria for EHRs and other HIT systems.

While these policies and regulations have been instrumental in driving the adoption of HIT, there is still room for improvement. In collaboration with healthcare organizations and stakeholders, the federal and state governments can take additional steps to address medical errors and promote patient safety. One potential area for improvement is the development of comprehensive patient safety programs and regulations that mandate the implementation of proven HIT solutions, such as CDSS and medication management systems. Governments can also provide additional funding and incentives for healthcare organizations to invest in these technologies and ensure proper training and implementation $($Karamagi et al$.,2023).$

Additionally, there is a need for more robust data$-$sharing and interoperability standards to facilitate the seamless exchange of patient information across different healthcare providers and systems. This can help reduce communication breakdowns and ensure that healthcare professionals have access to comprehensive and up$-$to$-$date patient data, which can ultimately reduce medical errors. Furthermore, increased emphasis should be placed on promoting a safety culture and continuous quality improvement within healthcare organizations. This can be achieved through education, training, and the establishment of robust incident reporting and analysis systems that enable healthcare organizations to learn from past errors and implement effective preventive measures.
