Major Transition Services Project in Nebraska

• What are the services and programs needed in my community, area or region?
• How can I meet those needs by developing a program or service to meet those needs?
• What is the very best approach to help children, students, adults transition from one program to another.?
• You are encouraged to survey the existing programs and services before designing your own transition plan utilizing existing services or collaborating with those that are in existence.
• Please take time to research your own community to determine what is already being used and whether it is successful.
• Utilize your observations, personal experience, interviews and hands on interactions with children, parents, students, and schools will help in your design of a perfect, ideal plan for your program.

Inclusion of current model(s) for transition services in CTE - (refer to your textbook for examples) Themodelincludes concept, purpose, content, educational activities, material and learning resource, measurement and evaluation method. Multilevelinclusiveeducation is co-learning in regular classes or learning environments of students with different classes, ages, and abilities, to serve the diverse characteristics of students. It is important when including information on the model that youexplore how current programs are operating and what they consider to be their model for inclusive practices.For example: In early childhood education, we follow anatural, inclusive model focusing on coaching the parentsinthe home which is the primary environment for learning and a child care environment supporting best practices.
3. Identify agoal, objectives and expected outcomes--(Note: This is also included in Part 3 - Plan of Action) a.Identify aspecial need(s) or disabilities to study- Ex. Downs syndrome b.Research thehistory, characteristics and backgroundof the special need or disability associated with a specific case study c. Gather afamily story or storiesbased on your experience working with parents/guardians that support the special need or disability.Note: You will be asked to use at least one family story as part of the field experience interview assigned in Part 2 of the project below. d. Become familiar with the IFSP and IEP process. e. Identifycurrent appropriate transition services based on legislative guidelinesneeded by the child/student and family from early childhood through adulthood including skill based and work based services. Ex. Early Development Network f. Become familiar withcurrent practices, policies, procedures and guidelinesfor providing transition services to meet the requirements for regulations associated with Part C: IDEA, disabilities and current legislative guidelines. g. Examinecurrent, relevant inclusive practices and transition servicesassociated with Special Education and CTE in your school, community, region, and state. - Include any examples of documents or forms used for the special services or programs. h. Develop awritten document that shares your research listed above A - Galong with your vision for best practice involved with the process of transition services from early childhood through adulthood including skill based and work based learning.
