Mr. A Background Mr. A is a 42-year-old divorced man assessed at intake to a crisis intervention unit. He was brought to the facility by the local law enforcement agency after his father complained that Mr. A had become paranoid and was threatening to harm family members whom he believed were plotting to kill him. The intake assessment worker described Mr. A as presenting with paranoid delusional beliefs and possibly responding to internal stimuli. She also noted that Mr. A demonstrated signs of disorganized thinking, frequently losing his train of thought, and that his speech was marked by clang associations. Mr. A has a long-established diagnosis of schizophrenia with paranoid features. He has been receiving case and medication management services while living in the community. He had several prior inpatient hospitalizations when his condition became exacerbated, typically when he discontinued taking his medication against medical advice. He had been unemployed most of his adult life and was living with his elderly parents. Mr. A was married for a short period of time during his early 20s. However, his wife sought and obtained a dissolution of their marriage when he began showing early signs of a serious mental disorder. Mr. A does not have any children.
