2. You have completed the Human Subjects Training - this training is required for all who work with human subjects.

3. There is no funding information required - no financial information needs to be provided.

4. The Informed Consent form has been completed and submitted for approval.

5. The data gathering instruments (surveys, checklists, etc.) have been completed and submitted for approval.
