Occurred: Patient X was seen in the outpatient clinic for severe abdominal pain, bruising, and swelling after abdominal surgery six months prior. The patient was given pain meds and stool softeners to assist with the pain however patient stated that the stool softener made the pain worse. Three months ago, the patient went septic and went into septic shock. When the abdomen was reopened, it was found that the sutures used to close the abdomen were permanent sutures and not dissolvable sutures. Also, there was a gauze pad found left in the patient.
