In one instance, a hand written receipt with Smith's signature was given to a patient who paid $15,000 in cash for a procedure. Later only $5,000 was applied to the patient's account in the billing software and only $5,000 in cash was deposited into the bank 10 days after the receipt date (no other bank deposits were done during those 10 days).
