On the infol.html page, add a form at the location of the word, Form. Inside the form, add two fields: a text field with the label Name, and a drop-down list with the choices of TV, Radio, and Web. Save the document when you are done. 1. Confirm that you are in Code-Design view (View > Split >> CodeDesign) 2. In Design view, select the word, Form 3. Click the Insert menu, hover over Form and select Form 4. In Design view, click inside the new form 5. Click the Insert menu, hover over Form and select Text 6. In Design view, change the new Text Field label to say Name 7. In Design view, place your cursor after the input box of the 'Name' label 8. Create a new line below the 'Name' label/text field combination by pressing SHIFT+ENTER (CMD+ENTER on Mac) 9. Click the Insert menu, hover over Form and then choose Select 10. In Design view, click the new Select dropdown field and then right click it and select Properties to open the properties window 11. Click the List Values\&utton in the Properties panel 12. Click in the Item Label input box and type TV, then click in the Value input box and type TV 13. Click the + sign 14. Proceed to add the word Radio for an item label AND value 15. Click the + sign 16. Add the word Web for an item label AND value 17. Click the OK button 18. Click the File menu and select Save
