Outcomes data used in quality measurement is often difficult to obtain and interpret. The data has several problems including all of the following problems, except:

A. Clear data definitions do not always exist

B. Patient populations are disparate and are difficult to use in comparing outcomes

C. Outcomes data is often harvested from clinical data sets which are used only for billing submissions
