My project is about :

Improving communication among employees and clients, Improving the method of documentation in clients as well as staff, making sure that the information entered is accurate, and lastly keeping a log of patient interactions in order to produce better health outcomes and reduce readmissions.

The EHR system is the phoenix system

The key project activity will be:

Designing a digital template that will allow the nurses to have clear and accurate communication among each other and their clients and this will help to alleviate the lack of communication/ misinterpretation between nurses and clients at the nursing facility.
