Overview of Course Project (Weeks 1-4). In this course, each week assignment will form a piece of a course project in which you develop a change initiative and action plan related to a health need within the community. In Week 1, you will identify a community health need. In Weeks 2 and 3, you will conduct a needs assessment to identify resources available to address the issue and gaps in the resource availability and you will select a partnership model or collaborative strategy and plan out a change initiative. In Week 4, you will develop an action plan to address the issue and resource gap and reflect on the process
