Case (The tragedy of Columbia Space Shuttle): In February 2003 the Columbia space shuttle disintegrated over Texas during re-entry to the earth's atmosphere. All seven crews died. In August an official inquiry was severe and unequivocal in its condemnation of cost cutting that took no account of safety requirements.
