Part 3: Reflection questions

To complete this part of the assessment, answer the following questions about the case management processes you facilitated.

It is important that you maintain peoples' privacy and confidentiality by not using any person's real name or other identifying details.

Your TAFE NSW assessor will mark this part of the assessment. It does not need to be signed off by the workplace supervisor. Read each question carefully. Your answers should follow the suggested word count for each question.

1. Choose one person you facilitated the case management process for. Briefly describe the outcomes of this process. Explain the impact of your own, the person's and at least one other key stakeholder's value systems on the outcomes. Your answer should be a minimum of 150 words and no more than 200 words.
