What do I need to do?

You are required to access external sources of information to research five (5) most common workplace accidents and/or incidents for your industry/type of workplace.

Once you have identified the five (5) most common workplace accidents, you are required to assess each risk and document your findings in the Risk Management Plan Download Risk Management Plan template provided. You must include the following:

• Identify the hazard - details of the accident/incident
• Assess the risk - details of what might go wrong or who may be hurt and how
• Control the risk - what actions can you take to prevent the risk from occurring, who is responsible for controlling this risk

How will I be assessed for this task?

For this task you are required to submit the following:

• Completed Risk Management Plan
• Use the Risk Management Plan template_V1.1 Download Risk Management Plan template_V1.1provided for you

Part B

What do I need to do?

You are required to conduct an audit of your workplace or if that is not possible, an environment with sufficient scale and operational diversity with the objective of identifying potential for risks to the business including injury or ill health. Minimum three (3) potential risks must be documented. If you cannot identify three (3) potential risks within your workplace, you can research and use an example. This example cannot be the same as in question 1. When conducting the audit, you must take into consideration the following:

• Risks other than Workplace Safety that may affect the business
• The physical working environment
• Task demands
• Workforce structure
• How work is organised

Conduct a comprehensive risk assessment of the potential risks you identified in your workplace. Take into consideration the cultural profile of the workgroup when carrying out the risk assessment.

Use the Risk Management Plan templated provided to document the following:

• Three of business risks Identified in the risk assessment
• Each hazard and their associated risks
• The Risk Rating
• The current controls for each risk (where they exist)
• The adequacy of current controls in relation to relevant standards (i.e.: Australian standards, industry standards and codes of practice)
• Which hazards require priority due to gaps identified between current control measure and relevant standard/s.
• Who in the workplace is responsible for controlling the risk and any relevant due dates

Part C

Using the Safe Work Method Statement (SWMS) template Download Safe Work Method Statement (SWMS) template provided, research and select three (3) high risk construction work activities and complete the SWMS for each activity. SWMS must include the following:

• The organisation (PCBU if in a WHS jurisdiction)
• The Manager in charge
• Date the SWMS was provided
• The site (or workplace location)
• High risk work activity
• Person Responsible for compliance with the SWMS
• Measures in place to ensure compliance with SWMS
• Person responsible for reviewing control measures
• Dates for receiving and reviewing SWMS
• Specific tasks involved in the high risk work
• Hazards and risks for each task
• Control Measures
• Worker Names and signatures.
• outline the actions required to implement the changes
• outline the expected reduction in risk as a result of the introduction of the control options

Seek authorisation and sign off from relevant stakeholders to implement your controls and to procure any resources required for their implementation
