4. Explain where the internal control structure breakdown occurred.
5. What if any fraud prevention and detection activities were in place prior to the fraud?
6. What was the predication that led to the fraud being investigated?
7. What red flags were present?
8. What types of evidence were gathered?
9. What legal action was taken and what was the result?
10. If you were the internal auditor for this company, what suggestions would you make to prevent future frauds from occurring?
