Transmittal document (Optional): This memo or letter is no more than one page in length, and it is to be addressed to the individual who requested/authorized the report (should be a separate page). The transmittal document is to have a single author (not the entire team), usually the most senior member of the report team. Your transmittal document will include a formal salutation, subject line, a statement to thank the board for reviewing this report, the purpose of the report and what they will find in the report, acknowledgement of the other team members' contributions, and a final paragraph (call to action) to provide context around any follow-up questions (or meetings) or if you need approval by a certain date. The author's signature block only includes information that was not included in the address block at the top. Enclosure refers to the name of the document that the reader will find attached.
