Task 1:
- The database and your final design document will require 3 tables. Using your knowledge of normalization and the business requirements, please analyze the data fields below to determine which fields belong in which tables. DO NOT create any additional fields or tables.
- Use the provided design document/template to complete you design prior to creating the access database.
- Include these tables and details in the design document/template.
Task 2:
- Use the provided design document/template to complete you design prior to creating the access database. Below is a list of the required data fields for the Database. Make sure your data fields are grouped appropriately within each table. For example, Customer information is not within the billing table.
Task 3:
- The primary key fields have been identified with an *. Assume the invoice number and appointment number will be the same. The primary key fields have been identified in your design document/template and within your access database.

Task 4:
- For each field define an appropriate data type. For example, fields containing dates should have a date data type. Fields containing names should have a data type of text.
Task 5:
- Your design should include relationships between the tables. For the three tables you should have two relationships defined, one between each table. (one-to-one, one-to-many or many-to-many). Referential integrity must be applied. Include your relationships in the design document/template and within your access database.
Task 6:
- Each field defined within each table should have a suitable field size. Know your business. For example, a First Name field should not default to a text field size of 255 characters. A length of 30 is more appropriate.
Task 7:
- Using what you have learned on how to import data into an access database using an excel spreadsheet, create 5 rows of data for each of the three tables in an excel spreadsheet. Then import this data into your database. Submit the spreadsheet with your assignment.
Task 8:
- Create a simple form for entering data into the Customer Table. Using your form, add 5 records into your Customer Table. Your table should have 10 records in total.
