Hospital X,Y, and Z is concerned about Patient Identity Management and knows that multiple departments will be involved in this process from the time a patient comes to the hospital through discharge and even post-discharge. You as the HIM Director have been given the task to write a policy and procedure on the Patient Identification Process. The policy and procedure must include the following:

• how patients will be identified when they come to the hospital (including when the patient does not have a government-issued ID)
• how patient identity will be managed for aliases for high-profile patients upon patient request, including the safety of the patient who may have multiple medical records in an electronic record system
• how matching newborns with mothers will occur
• how clinical information will have proper patient identification (including the medical record, medical record forms, laboratory results, radiology reports, etc.)
• how e-health delivery manages patient identity issues
• what the process is for identifying patients for release of information requests and accounting of disclosures
• what the process is for patient identity at the time of surgery
• how the identity of the patient will be protected in an electronic record system
• how to prevent and mitigate identity theft
• how other patient identity issues will be addressed
• how patient safety is increased with the use of patient identity measures

Instructions:

1. Evaluate Joint Commission, Conditions for Participation in Medicare, Kentucky State Requirements, and HIPAA for patient identity management requirements. Provide written documentation of specific laws and regulations pertaining to patient identification and the value of the law and regulation in improving patient safety and the effectiveness and efficiency of the health care delivery system.
2. Determine which departments would be involved in the patient identity management process.
3. Write a policy and procedure for Patient Identity Management based on the above.
4. After completing the policy and procedure, review the following situations and apply the policy and procedure you wrote to the situations providing written resolutions to the situations on Blackboard.
5. Determine if the policy and procedure document needs revision based on applying the P&P.
6. Make any necessary revisions to the policy and procedure document.

Patient Identity Management Situations:

1. The driver of a motor vehicle accident drives himself to the emergency room with minor cuts and abrasions. He is unable to provide any form of identification but tells the registration clerk that his name is James Montgomery and he was at the hospital two years ago for an appendectomy. How do you as the registration clerk proceed in this situation?

2. The radiology department takes X-rays and hand writes the name of the patient on the x-ray once the x-ray has been processed. You are the HIM Director and you recognize that there are several instances where the x-ray could be misidentified. What process will you use to resolve this issue?

3. Tom Cruise is in town making a movie when he wrecks his motorcycle. He is brought to your hospital with a head injury. How do you protect his identity and ensure that information is not leaked to the press?

4. A former patient comes to the hospital and wants an accounting of disclosures of release of information. He shows you his drivers license but he looks much older than the date of birth in the computer system. How do you handle this?

5. The hospital is developing a patient portal. How will patient identity be ensured upon entry into the system and between applications within the patient portal?
