There is such a large range that patients can inhabit as far as the level of care they need and that can be very different when considering LPN and UAP duties. A patient can be totally dependent with transfers and perineal care but be relatively easy when it comes to medication management. Likewise, you can have a patient that is nearly independent but have a wide range of skilled cares that they require from lab draws, medication administrations, and discharge teaching that would take a lot of a nurses time and none of a UAP's.

Important information to know would be any health conditions the patients have and allergies. This is important to know because medications play a big role into the patient's conditions. For example, if the patient has impaired renal or liver functions, then there would be a lot of medications that would be need to be adjusted or avoided. Allergies are also an important factor to know for the RN in case their allergic to meanly medications or certain materials such as latex.

It's imperative to be efficient and correct when passing down SBAR report to providers and next shift nurses.
