• The HCCB Plan must be visionary and include worksheet items such as,

  • Current State,
  • SWOT Analysis
  • Gap Analysis
  • SMART Goals (Personal & Professional), Strategies, Tactics, and Success Metrics. (SEE RUBRIC)
• The HCCB Plan MUST include at least three (3) personal goals AND three (3) professional goals, Each goal must include at least three (3) strategies and three (3) tactics.
• The HCCB Plan is a reflection of you and should be presented in a professional manner.
• The HCCB Plan should include narrative, descriptions, detail, and rationale. Bulleted listingsmustinclude a detailed narrative.
