2. Swim Lane Diagram: How would a swim lane diagram of the MyChart eVisit workflow look like?

 

This case study walks through the workflow for implementing eVisit functionality in MyChart, the health system's patient portal.  This workflow is a new feature that the health system is implementing.  Currently, patients are seen in person following the standard appointment scheduling process.


E-visits allow patients to communicate with nurse practitioners virtually instead of coming to the clinic only for specific reasons. Do not charge the patients until the visit is completed. $30.00 hold to process the e-visit and if the patients do not come in, they do not get charged. Then they would pick a reason for a visit with 9 options. Only allowed one symptom per visit. If they have more than one visit, they would have to visit the clinic. The nurse practitioner is allowed to prescribe them medication and patients have to pick the location wished for medication to be sent. Patients view their demographic information to make sure everything is still correct. They have the opportunity to review their health problems to make sure everything is correct. They can indicate if they have any allergies and have some selections already to choose from. Lastly, there is a questionnaire to ask patients if they have any other symptoms which will force them to come in to get checked. If the patient selects one of the options, then they would have to come in but if the patient says no to any of the visits, they will continue the questionnaire. The questionnaire was embedded by a team of providers. Some of the questions want the patient to elaborate so they will have more options to choose from. The questions are related to the reason for scheduling an e-visit. In the end, it gives the patient an option to choose from if they are breastfeeding, or pregnant. Also, there is an open field to allow patients to type anything else that the nurse practitioner would need to know. There is an option to allow patients to upload an image or document if needed to help providers. They have the opportunity to review everything before submitting the e-visit. They are allowed one day to hear a response from a nurse practitioner, but the goal is 4 hours. Once they have all the information, they can place orders for medication and can place orders for labs. The patient can do that for convenience from home. 

 

Key notes: Only goes to 3 nurse practitioners. 3 nurse practitioners monitor the pool. Only for primary care.
